I am a board-approved supervisor for LMSW social workers in Texas seeking their LCSW clinical licensure. I practice from a relational-cultural and attachment theory perspective and have experience working with children, adolescents, families, and adults in inpatient, intensive-outpatient programs, and traditional outpatient settings.

I have a special passion for creating succinct, organized, and formatted documents and policies for my clients and myself that help me communicate clear expectations and boundaries about the services clients invest in with me as their therapist. These downloadable forms are ready to input your own practice name and are available to edit as you need. The legal forms have been reviewed by an attorney who specializes in Mental Health Professional Practice Compliance as recently as May 2022. Forms and Bundles include:
